Sacred Hearts Transitional Housing Application Process
At Sacred Hearts Mentoring Program, we are committed to providing a safe, supportive, and structured environment for re-entry citizens and single mothers who are seeking stability, restoration, and a fresh start. Our application process is designed to ensure that every participant receives the support and resources needed for long-term success.

Individuals interested in transitional housing may contact Sacred Hearts through our website, phone, email, referral partners, churches, community organizations, correctional institutions, or outreach events. During the initial inquiry, applicants will receive general information about housing availability, program expectations, eligibility requirements, and supportive services offered.

Applicants must complete a Transitional Housing Application Packet. The application may include:
Personal identification information
Emergency contact information
Current housing situation
Employment and income status
Criminal background information (for re-entry applicants)
Family and dependent information
Medical and mental health disclosures (if applicable)
Personal goals and support needs
Applicants may also be asked to provide supporting documents such as:
State-issued ID or driver’s license
Social Security card
Birth certificates for children (if applicable)
Proof of income or benefits
Release documentation (for re-entry citizens)
Referral letters or supporting documentation

Step 3: Intake Interview
After the application is reviewed, eligible applicants will be scheduled for an intake interview with Sacred Hearts staff. This interview helps assess the applicant’s immediate needs, goals, barriers, and readiness for transitional living. Staff will also explain program guidelines, curfews, participation expectations, mentoring requirements, and community standards.

Step 4: Assessment and Program Planning
Applicants may complete additional assessments related to:
Mental health and wellness
Employment readiness
Educational goals
Life skills development
Substance abuse recovery support (if applicable)
Parenting and family support needs
Based on the assessment, Sacred Hearts staff will develop an individualized success plan outlining housing goals, employment or educational objectives, mentoring support, and personal development milestones.

Step 5: Acceptance and Orientation
Approved applicants will receive notification of acceptance and move-in availability. Before placement, participants must complete a program orientation covering:
House rules and expectations
Safety and security procedures
Case management services
Mentorship participation
Financial responsibility guidelines
Conflict resolution procedures
Community engagement expectations
Participants may also sign residency agreements, consent forms, and program participation documents.

Once orientation is complete, participants will move into Sacred Hearts transitional housing and begin receiving supportive services. Residents will work closely with mentors, case managers, and program staff to achieve personal, financial, educational, and housing stability goals.
